The Health Information Services Department at Rhea Medical Center helps to produce, maintain, retrieve, and protect confidential health information for all hospital patients. We strive to produce quality information, code each record accurately, and release information following strict guidelines.

The availability of high-quality, complete, timely, and accurate information to healthcare providers, patients, and other users of healthcare data is of the utmost importance. We also strive to work with physicians, hospital staff, patients, and other customers in an effort to benefit the healthcare consumer and promote the mission and values of Rhea Medical Center. The Health Information Services Department here at Rhea Medical Center is proud to have a competent staff with a combined total of over 50 years experience.

Confidentiality and Medical Records

The health information staff takes confidentiality and security of each individual’s health information very seriously. You have the right to review and obtain copies of your medical record. Requests for these records are handled in accordance with Tennessee State Law, Federal Law, and the policies and procedures and Medical Staff Bylaws, Rules, and Regulations of Rhea Medical Center.

Hospital policy requires a 24-hour notice for release of information. You must bring picture identification with you to obtain a copy of your medical records. You must sign a release of information to obtain or release your records (see below).

No one else can sign a release for your records except you, including your spouse. Only natural parents or legal guardians (must show evidence) can obtain a copy of their minor children’s records. A Durable Power of Attorney for Healthcare is required to obtain copies of records on anyone other than you. Also, legal documentation must be presented to obtain records on a deceased patient. In some instances, there may be a charge for a copy of your medical records.
